Live Well Dementia

This work describes a vision to support people to live well with dementia in their communities and a place they call home.

This offer would best support people with dementia who are living in their or their loved ones’ home, either independently or with support from family and/or caregivers. This complements other work led by Dementia United that focuses on the earlier and/or later stages of a person’s journey and disease progression, including palliative and end of life care.

As part of future development of this work, Dementia United will continue to explore and define how this work integrates with their other support offers within different stages of a person’s journey.
